To authenticate to a remote SMTP server, use hosts_require_auth in
your transport configuration, e.g.

auth_smtp:
    driver = smtp
    interface = 10.0.0.1
    hosts_require_auth = host1:host2


and in the authenticators section, specify your driver:

fixed_cram:
    driver = cram_md5
    public_name = CRAM-MD5
    client_name = auth-user
    client_secret = password


This worked for me using Exim as an inbound gateway.
